Polk County High School will offer athletic physicals on May 2 for any athlete planning to participate in sports during the 2024-25 school year.
Physicals will be offered from 2-5 p.m. in the PCHS gym lobby. Athletes must have a physical on file before being allowed to practice or participate in any other team activities.
Cost is $25, and paperwork must be completed by parents/guardians prior to arrival. Forms may be downloaded or picked up in the school office at PCHS or Polk County Middle School.

Students who turn in their completed form to PCHS Athletic Trainer Jason Nussbaum before May 2 will be entered into a drawing, with one male and one female athlete being chosen to have their fee waived.
