PCHS students must complete registration form to participate in athletics

Any student interested in participating in athletics at Polk County High School during the 2022-23 academic year needs to complete an athletic registration form.

The form can be accessed on the school athletic department website.

Student-athletes must also have a valid physical on file with the athletic department.
