AdventHealth to offer sports physicals at PCHS on May 1

AdventHealth providers will offer sports physicals for all school-aged athletes on Friday, May 1 at Polk County High School.

The physicals will be available from 1:30-4:30 p.m. in the PCHS gym lobby for $25.

Parents must complete and sign all forms in advance. Forms can be printed at https://polkschools.org/physical or picked up from the main office at Polk County High School.

Payment by cash, card or check (made out to PCHS) will be accepted.

All athletes must have a physical on file at their school before being allowed to practice or play in any sport.
